Seaworthy form help

Hi

Can someone please provide me with a link of the "12 page form" I must fill in for my samsa seaworthy certificate. I have a 4.1 m category E rubberduck.

The lady at H2O inflatables (couldn't understand A WORD she was saying) said that I must fill in a 12 page form that I must print from the samsa website and bring her when I go for my certificate of fitness and buoyancy, but I can't find it anywhere on the internet.

Do they ask for your skippers license and license of trailer when the survey your boat?

Ripper-za, take your boat to a SAMSA registered examiner for the buoyancy certificate. He will fill in the forms you are referring to. It is not your job to fill them in. That woman certainly does not know what is going on.It would be advisable to take your license and other forms with in case they ask for them.

Thanks, but the thing is that H20 is one of the main SAMSA accredited buoyancy/seaworthy fitness examiners in the cape and it is also the one closest to me.

The women is afrikaans but she has a slurred speech and on top of that talked with a low voice. She has been working there for a few years now working with boats daily. The other thing is, the guy I bought the boat from fitted the hull with their h20 pvc pontoons and all the previous checks were done at h20, according to the seller and I can see on it's past papers.

The only thing that I clearly understood, and I asked 3 times to make sure, was the bit about the 12 page SAMSA form I have to type out and print so that I can take it to them the day, because it has to be sent to samsa. She gave me samsas website address, but I can't find it on their anywhere.

this is all about the buoyancy how to calculate etc.

if they fitted the h2o pvc pontoons, they should submit you with a certificate where it states all

If they are accredited by SAMSA then they should have the forms to fill in.This is the first time that I hear that you have to supply their forms. The official that surveys my boat supplies his own forms, I've just got to take the boat to him and he does the rest.

This is the form that you need to fill in. It strange how the shop can fill in the form if you must fill personal detail. I have done a few boats at H2O. They know what they are doing.
